Payment for Out-of-Network Care

A maximum allowable fee is defined as the difference between the amount Dean Health Plan pays and the amount that the provider bills. You may be responsible for the difference. This includes follow-up care that was prior authorized. Please reference your Group Member Certificate and Benefit Summary for more information.
