Tobacco-Cessation Benefit
As part of your health benefits, Dean Health Plan will provide you
with any of the smoking-cessation medications in our drug formulary
with a prescription from your doctor for the cost of a generic copayment
or $10 per fill, whichever is less.* Tobacco-cessation medications
include bupropion, Chantix or over-the-counter nicotine-replacement
therapy. To obtain a prescription or for more information about overthe-
counter therapy, members should call their healthcare practitioner
to discuss appropriate treatment options.**
* Copayments listed are good through
December 2007 and are subject to change.
** Dean Health Plan members who receive
benefits through state employment, federal
government or the Medicaid/BadgerCare
program have different tobacco-cessation
