If you have, there is a good
chance it came from the DHP Care
Management Program. The Care
Management Program was created
to improve the health of our members
by addressing health issues at
the earliest possible stage.
Help for Those Who Need It
The DHP Care Management Program assists members who are at a higher risk for certain conditions, diseases or complications. The program also focuses on measures that help prevent diseases and conditions that may result from missed services or nonideal health behaviors.The number of mailings you receive depends on a few factors. A member with more conditions, such as diabetes and heart disease, who has a higher number of missed tests or services, such as a blood pressure screening or a flu shot, will receive the most materials. Those who do receive these services will receive fewer mailings.
A Complement to Your Care
It is important to understand that the Care Management Program is meant to make you aware of health-related topics. It does not replace the advice of your primary care practitioner (PCP).
