Being a Dean Health Plan Member

Photo of man smilingYou deserve the best service and healthcare possible. It is important to us that your rights as a member of Dean Health Plan (DHP) are respected. It’s equally important that your responsibilities as a member are explained.

Your Rights
As a DHP member, you have the right to:

  • Be treated with dignity, respect and compassion regardless of race, color, age, sex or creed
  • Receive a list of DHP participating practitioners to help you choose your primary care practitioner
  • Present a question, complaint, appeal or grievance to DHP without fear of discrimination
  • Be given information on procedures and policies about your healthcare benefits
  • Get timely responses to requests about your healthcare plan
  • Request information about advance directives
  • Participate with practitioners in decision making about your healthcare
  • Have a candid discussion of appropriate or medically necessary treatment options for your condition, regardless of cost and benefits coverage
  • Receive information about the organization, its services, its practitioners and providers, and members’ rights and responsibilities
  • Make recommendations regarding the organization’s members’ rights and responsibilities policies

Your Responsibilities
As a DHP member, you have the responsibility to:

  • Read and understand the materials provided by DHP concerning your healthcare benefits
  • Contact us if you have any questions
  • Present your ID card to identify yourself as a DHP member before receiving services A Notify DHP of any enrollment status changes, such as family size or address
  • Provide complete information to those providing your healthcare
  • Cooperate with and follow the advice that you and your healthcare practitioner have agreed on.

We encourage you to ask questions if you do not fully understand the instructions given. A Understand your health problems and participate in developing mutually agreed-upon treatment goals to the degree possible
