Dean Health Plan is pleased to announce a change to our member documents that will make it easier for you to find and understand your benefits information.
Currently, your policy consists of two separate documents, the Member Certificate and the Schedule of Benefits. You need both of these documents to fully understand your health care benefits, making it a logical step to combine these two documents into one simple-touse document called the Member Certificate and Benefit Summary. This will make it easier for members to organize and understand their benefits information. It will be available to you when your policy renews in 2005.
If you are a State of Wisconsin or federal member, your documents will remain the same because your employer uses uniform documents for all employees.
For your convenience, you can find your policy information online at www.deancare.com by following these easy steps:
- Choose “Insurance Services.”
- Choose “For Members.”
- Choose “Your Benefit Information” from the drop-down menu.
Your policy can be easily printed from our Web site or, if you would like us to mail you a copy, call 800-279-1301 (press 3 and follow the prompts). We will be happy to mail it to you right away. We invite you to take a look at the new policy format and welcome any feedback you may have.

| Cochlear implants | These will be covered for group policies. Services must be prior authorized and are subject to policy provisions. |
| Emergency room copayment | The copayment will be waived if a member is admitted as an inpatient through the emergency room. |
| Growth hormones and Ceredase | The appropriate copayment will now apply instead of a 50 percent copayment. |
| Hearing aid repair | Repairs will be applied to the hearing aid benefit maximum. |
| Outpatient MRI/CT scan copayment | The copayment for MRIs and CT scans will be $50 per visit instead of per scan. The maximum out-of-pocket cost is $150 per member per contract year. |
| Choice plans | Covered services that we cannot provide “in plan” will be paid at a plan benefit level instead of a nonplan benefit level. |
| Volunteer-, education- and travel-related immunizations | These will be covered. |
| Prescription drug benefit | The prescription drug benefit maximum has been removed. |
