Please let us know if your address changes. We mail updated benefit information, ID cards and our newsletter to the address that you provided to us. If we don’t have your most recent address, you may not receive important information. You can call our Customer Service Department or leave a message with your updated information on our automated voice response system at 800-279-1301.
As a member, it is also your responsibility to notify us of any changes that might affect your coverage. You should report these changes to your employer’s insurance representative or to us as soon as possible. These changes include, but are not limited to:
- Eligibility for Medicare
- Loss of eligibility for coverage due to termination of employment, divorce or death of the subscriber
- The addition of newly acquired qualified dependents
- For qualified dependents over age 18, reaching the maximum dependent age or loss of full-time student status
- A change of dependent eligibility to include termination of parental rights
