Dean Health Plan is conducting its annual full-time-student mailing. We use this questionnaire to determine whether we can continue to cover dependents who are full-time students. The mailing is sent to members who have dependents ages 19 and older.

If you receive a questionnaire, please complete it and return it to our office by the due date listed. This will help ensure that our records for your dependent are up-to-date. If we do not receive the questionnaire back, we will assume that your dependent no longer qualifies for coverage, and his or her coverage will end.

As a reminder, in order for your dependent to be covered as a full-time student under Dean Health Plan, he or she must meet the following criteria:

• Be a qualified dependent (biological, adopted or stepchild).
• Be a registered full-time student at an accredited post-high-school academic, professional or trade school. This generally means that he or she must earn 12 or more credits per semester.
• Provide less than 50 percent of his or her financial support.
• Be unmarried.
• Not be on active duty in the armed forces.
